Here is a link to Health & Well-Being details. Submit your Acknowledgment Form for the Addendum to the Family & Student Handbook. The form must be completed in order for students to attend the first day of school.
Complete the Summer ePacket action items.
Send a copy of your child's current Georgia Certificate of Immunization Form 3231 (provided by your child's pediatrician) to Nurse Lindsay Huie at email@example.com.
Submit the Transcript Release Authorization Form to your child's current school to request final reports be sent to Mount Vernon.
