Schedule an appointment for your child to see one of our staff members for an injury evaluation, consultation, injury treatment, or rehabilitation.
Pre-Participation Physical & Consent Forms
Above, you will find the link for the GHSA Pre-Participation Physical & Consent Forms packet required for athletic participation at Mount Vernon School. Please complete the form packet prior to your child’s physician appointment, bring your completed form packet with you to your child’s physician appointment, then upload your child’s completed form packet using the Lookup portal link to submit your child’s forms to the MV Sports Medicine Department. Do not submit these forms to a coach or other faculty member of Mount Vernon School; these forms should be submitted directly to the MV Sports Medicine Department. Photo submission of these forms will not be accepted; only a scanned PDF may be submitted for validation purposes.
Please note, the MV Sports Medicine Department is not able to conduct a physical examination; this must be completed by a MD, DO, NP, or PA. Additionally, it is important to note that a pre-participation physical is different from an “annual wellness” exam, as such, there may be an additional cost not covered by your insurance plan. This physical is a functional assessment to determine if your child is physically capable of participating in athletics.

Understanding Concussions
Above, you will find the link to schedule an appointment with the MV Sports Medicine Department to request your child be evaluated for a concussion. Please note, concussion may take up to 72 hours before they fully present; however, early recognition and diagnosis is key to implementing the proper recovery procedures. At Mount Vernon School, we have established a Concussion Response Team (CRT) dedicated to managing your child’s concussion recovery from a Return-to-Learn (RTL) and Return-to-Play (RTP) stance.

Sudden Cardiac Arrest (SCA) is a condition in which the heart unexpectedly stops beating. SCA is usually caused by an electrical disturbance in the heart. SCA results in death if not treated within minutes.
